Feedbacks are usually taken after completion of a course, training program or workshop. This is a procedure that helps the organizers or trainers to know whether the participants have understood and liked the training. Also, the feedback helps them to make improvements in the areas of training that are not up to the mark.

• This sample sheet is a post event evaluation sheet for an instructor led training workshop. The purpose of this form is to gather information to improve the training program.
• The parameters have to be judged in the range of 1-5, 1 being strongly disagree and 5 being strongly agree.
• The sheet is divided into six sections: learning effectiveness, program information/content, presenter, job impact, support tools and net promoter.
• Each section has one or more questions. All these questions are to gauge the answers about the program, content, the presenter and how the training would affect their work.
• After each section, a column is provided for their comments if their grade for that section is 2 or below.
